Tomii clients with this form disclosing our services, qualifications, and
the
restrictions that apply to our practice. This form describes both the nature
and
theory of our services, the educations and the qualification of your
personal
Tomii practitioner. Please sign below that you understand the information

Tomii Practitioner Qualifications: Tomii practitioners are
“certified massage
therapists”, which means that they are certified by the massage therapist
organization (CAMTC), pursuant to Chapter 10.5 of the California Business
and
Professional Code. They have also received intensive training (about 500
hours)
of the Tomii Healing and Wellness System, which includes Traditional Chinese
Meridian and Acupoints system, cupping, Meridian Qi Analyzer, etc.
The Theory upon which our services are based: According to
Traditional
Chinese
Medicine (TCM), the body has natural patterns of “Qi” that circulate through
the
body in meridians; several hundred acupoints can be located along these
meridians. If the movement of Qi becomes blocked, the health and well-being
of
the individual is adversely affected. Tomii practitioners target the
acupoints
and meridians to improve the circulations of Qi and create new Qi.
The Nature of Our Service: Tomii practitioners seek to free
energy pathways
within the body and strengthen the mind and body of the individual for pain
relief and energy restoration. We measure and assess your meridian Qi with
the
Meridian Qi Analyzer. The histogram report of the 12 meridians Qi and the
report
from the analyzer are only for reference for understanding the Qi balance of
your body. This aids the practitioner to massage specific areas to help
relieve
pain naturally. We may use deep acupressure massage with hot stones,
infrared
heater, herbal oils, to remove blockages and restore the flow of energy. We
also
utilize “chi cupping”. The skin is drawn up into the cup creating a seal.
With
oil the skin, the cups may be moved around the body along the meridians and
at
the specific points to promote the flow if Qi.

Tomii practitioners will not perform any form of surgery or
any procedure
that
punctures your skin or harmfully invades your body; use of x-ray radiation;
prescribe prescription drugs; recommend that you discontinue drugs that were
prescribed by a licensed physicians; set fractures; treat wounds, put you at
risk of great bodily harm; serious physical or mental illness or death.

therapy and bodywork offered by Tomii practitioners. I have discussed with a
Tomii practitioner the nature of services to be provided. I understand that
a
Tomii practitioner is not a licensed physician, and the massage therapy and
bodywork services are not licensed by the state. I understand it is my
responsibility to maintain a relationship for myself with a medical doctor.

I understand that cupping therapy typically causes dark mark reddening of
the
skin that may last for several days.
